THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION.

2. WHO WILL FOLLOW THIS NOTICE. The notice describes Ledwell Advanced Imaging system’s practices as well as the privacy practices of (a) any healthcare professional authorized to enter/access information into your chart; (b) All employees of Ledwell Advanced Imaging; (c) All departments of Ledwell Advanced Imaging; (d) All employees, staff and other advanced imaging personnel with access to protected health information (PHI); (e) All students (medical, nursing, lab, radiology, etc.) while they are participating in continuing education training.

3. PURPOSE OF THIS NOTICE. We are required by law to maintain the privacy of your medical information. We create a record of the care and services you receive. We need this record to provide you with quality care and to comply with certain legal requirements. This Notice applies to all of the records of the care and services you receive, whether made by employees or your physician(s). This Notice will tell you about the ways in which we may use and disclose medical information about you. This Notice also describes your rights certain obligations we have regarding the use and disclosure of your medical information.

OUR DUTIES: We are requires by law to:
a. make sure that medical information that identifies you is kept private;
b. give you this Notice of our legal duties and privacy practives with respect to medical information;
c. follow the terms of this Notice as long as it is currently in effect. If we revise this Notice, we will follow the terms of the revised Notice as long as it is currently in effect.

HOW WE MAY USE AND DISCLOSE MEDICAL INFORMATION ABOUT YOU. The following categories describe different ways that we use and disclose medical information. For each category of uses or disclosures we will explain what we mean and try to give some examples. Not every use or disclosure in a category will be listed. However, all of the ways we are permitted to use and disclose information will fail within one of the categories.
